CentraCare - PACE

Comprehensive Community-Based Older Adult Services

Senior Services of Southwest Michigan works in partnership with CentraCare to make valuable PACE services available to older adults and their families in our area. CentraCare is part of the National PACE (Program of All Inclusive Care for the Elderly) Association. PACE programs offer a medical/social-based model of care that provides comprehensive integrated managed care, on a voluntary basis, to older adults who are at risk for nursing home placement.

All services and products covered through Medicare and Medicaid are provided by the PACE organization in Battle Creek (At their site in the Burnham Brook Center), and soon services will also be available in Kalamazoo Michigan as well.
