Case Coordination and Support:

Case Coordination and Support Services, offered by Senior Services ensures that seniors receiving personal care, home delivered meals and other needed services are efficiently and appropriately served.

We do this by providing a comprehensive in-home assessment to determine client's needs and coordinate pertinent services.

Our Case Coordination staff develop a care plan with the client to identify and put into place those needed services.  Referrals are also made to other community agencies and resources to address emergency and basic needs related to housing, budgeting, medical access, income assistance and social or recreational opportunities.

Senior Services Case Coordinators also serve as the agency's central point of intake for those requesting services.
