The MI Choice Program:

The MI Choice program allows frail elderly and disabled person who are eligible for Medicaid funded nursing home services, to remain in their own homes.

Eligibility:

You must meet Nursing Home Level of Care criteria
You must meet Medicaid income and asset criteria
You must need one or more of the available services

Our Service Area:

The MI Choice services are available in Kalamazoo, Barry, Branch, Calhoun and St. Joseph Counties in Southwest Lower Michigan.  Senior Services staff provide continued assurance and monitoring of service quality throughout the client's enrollment in the MI Choice program.
